In 1996, Cao returned to Shanghai Shenhua as their head coach. He guided the club to another league title in 1997-98, but he was fired after only six matches due to poor results.

After leaving Shanghai Shenhua, Cao became the head coach of Jiangsu Urawa Red Diamonds. During his time with the club, he played a key role in helping them reach the finals of the Asian Cup in 1999-2000 and 2001-2002. Despite not achieving success at either tournament, Cao remains one of the most successful coaches in Chinese football history.
