Cao's coaching career began at Wuhan United FC, where he helped them win the Chinese Super League in 1984-85. He then went on to coach Jiangsu Urawa Red Diamonds, where he led them to their first ever league title in 1987-88.
In 1988, Cao was appointed as the head coach of Shanghai Shenhua. At the club, he had a significant impact on the team's success. Under his leadership, Shanghai Shenhua won the Chinese Premier League in 1989-90 and reached the final of the Asian Cup that year. However, the team struggled during the early part of the season, which saw them lose to Japan's Fukuoka Pro Heroes in the quarter-finals. Despite this setback, Cao continued to work tirelessly for the club, hoping to turn things around.
In 1992, Cao was appointed as the new head coach of Shenzhen Beihai Sports Club. In his first season at the club, Shenzhen achieved promotion to the China League One, but the following season saw them struggle to make it out of the relegation zone. Cao was sacked from the club after just three games in charge due to poor results.
In 1996, Cao returned to Shanghai Shenhua as their head coach. He guided the club to another league title in 1997-98, but he was fired after only six matches due to poor results.
After leaving Shanghai Shenhua, Cao became the head coach of Jiangsu Urawa Red Diamonds. During his time with the club, he played a key role in helping them reach the finals of the Asian Cup in 1999-2000 and 2001-2002. Despite not achieving success at either tournament, Cao remains one of the most successful coaches in Chinese football history.
